After scheduling & before your appointment:

  • Please check your email to ensure you have received a confirmation of your appointment. If you have successfully scheduled an appointment, you will receive an email confirmation with:

    • the date and time,

    • a link to complete the proper intake form,

    • directions on how to find me,

    • and instructions on how to prepare for your appointment.

  • Be sure to complete your intake form BEFORE your appointment if this is your first time scheduling with me.

  • Consult with your physician if you have any reservations or concerns.
